Internal Medicine and Primary Care

Internal Medicine encompasses comprehensive medical and preventive care for a wide range of conditions affecting our adult patients. Patients are seen on a regular basis for ongoing wellness care and to reduce the risk of cardiovascular disease, cancer and other serious conditions. Preventative medicine is a key component of our all-inclusive approach to good health. We will guide you through the complex cancer screening process, educating you on age-appropriate testing to maximize early detection and diagnose the presence of any disease before it has progressed.

We aim to provide prompt and effective care for patients with acute medical concerns so that they can return to optimal health and resume their regular activities. In most cases, we can offer same-day or next-day care. Patients see our general medicine doctors as a first line of defense for their medical needs, and may be referred to a specialist within one of our other departments if necessary.

Travel Related Illness

When traveling, especially abroad, individuals may be exposed to certain viruses, bacteria or parasites that they would not have come into contact with at home. Therefore, it is important to prepare for a trip with a visit to the physician in case any vaccinations need to be administered. Upon returning home, if a patient is experiencing a fever or uncomfortable symptoms, it would be beneficial to be evaluated by a doctor who can provide a treatment plan.

There are a wide range of illnesses that may be acquired by travelers, but by far the most common is traveler's diarrhea. Often due to the presence of different strains of bacteria than those typically found in the United States, diarrhea may occur in conjunction with nausea, vomiting and fever. Treatment is usually focused on providing medications to resolve the diarrhea as well as preventing dehydration.

Another common health issue for travelers is illness transmitted via an insect. Some of these may potentially be very serious, so it is essential to visit a doctor for testing in order to determine what is causing the symptoms. Many of insect-borne diseases require treatment with antibiotics or another form of medication.

Hotel Calls

Hotel calls are a service we provide to travelers seeking a convenient source of personal or urgent medical care. A physician will be available as needed to attend to patients at their hotel. We offer this option 24 hours a day through our live operator answering service. Diagnosis of illnesses and many treatments can take place on-site in the hotel room.

We are available daily for housecalls and office visits on an appointment basis and are available for our patients 24 hours through our 24 hour live operator answering service. In addition, should a patient require hospitalization, Dr. Primas will attend to them as an inpatient at two of Manhattan’s finest teaching hospitals.

Obesity & Overweight

Too often, people think they can lose weight quickly through brief, strenuous dieting or sudden spurts of exercise. In fact, however, a great majority of people who lose weight rapidly regain that weight within five years. More methodical, ongoing treatment is usually more effective and more long-lasting. Obesity is a serious problem. Overcoming obesity requires a commitment to lifestyle changes. There are several methods of treatment for obesity.

Proper Diet

Adopting good eating habits is essential in achieving a healthy weight. A safe and effective long-term weight-loss diet must contain balanced, nutritious foods to avoid vitamin deficiencies and malnutrition. A healthy diet should:

  • Be high in whole grains, fiber, fruits and vegetables
  • Contain lean meat, fish or vegetable protein
  • Be low in sugar, fat and fried foods
  • Include whole grains and high fiber
  • Eliminate fast food consumption
Physical Activity

Regular exercise is an important part of a healthy life. Physical activity and exercise help to burn calories. The number of calories burned depends on the type, duration and intensity of the activity. For some individuals, breaking down exercise routines into smaller sessions is helpful. Treating obesity with exercise is most effective when combined with a healthy diet since exercise alone will have a limited effect on weight loss.

Recommendations for healthy exercise habits include getting 30 minutes of moderate exercise five to seven days a week, starting slowly and progressing gradually to avoid injury, excess fatigue and body aches.

Prescription Weight-Loss Medication

Weight loss is best achieved through a healthy diet and regular exercise. For some individuals, weight-loss medications may be of help. Such medications are not usually prescribed unless diet and exercise have already been tried and failed. Even with prescribed medications, lifestyle changes must be implemented for obesity to be treated successfully.

Bariatric Surgery

When other methods of weight loss are unsuccessful, particularly when an individual suffers from obesity-related medical conditions, bariatric surgery may be considered. Since any surgery involves some risks, bariatric surgery should never be performed without serious discussion between patient and doctor.

Under the proper conditions, bariatric surgery may be a lifesaving procedure, though the patient will still be required to make permanent lifestyle changes. Before and after weight loss surgery, counseling is necessary to assist the patient in achieving and maintaining lasting positive results.
